The accident happened at the Easts Beach Caravan Park, on Ocean Street, just before 3.30pm.
Police have been told a woman, aged in her 30s, was reversing the car when the girl was hit.
Paramedics and doctors from the NSW Ambulance rescue helicopter worked for more than 90 minutes to stabilise the girl before she could be airlifted to hospital.
The girl has been flown to the Sydney Children’s Hospital at Randwick.
It is understood the girl’s mother is on-board the helicopter, which took off from the Bonaira Playing Field just after 5pm.
A NSW Ambulance spokeswoman said the helicopter crew was monitoring a storm in the Sydney area, which could see the aircraft diverted to another hospital.
The spokeswoman confirmed the girl was in a critical condition, with suspected chest and abdominal injuries.
5.10pm: The NSW Ambulance rescue helicopter has just left Kiama, where an 18-month-old girl was injured after being hit by a car at a nearby caravan park.
The girl, who is in a critical condition, will be flown to the Sydney Children’s Hospital at Randwick.
4.40pm: An 18-month-old girl has been critically injured after she was hit by a car at Kiama.

A crime scene has been set-up at the caravan park and police inquiries are continuing.
