Emergency services were called to the property on Walkers Road in Avoca, about 15 kilometres south of Bowral, at 9.15am on Tuesday to reports that a 69-year-old man had been injured.
A NSW Police spokesman said the driver of the vehicle was the injured man's wife.
Police were investigating whether there was a mechanical fault with the vehicle, or whether the woman may have accidentally pressed the accelerator rather than the brake, the spokesman said.
The accident occurred on private property, he said.
A NSW Ambulance spokeswoman said the man was suffering what appeared to be serious spinal injuries following a collision with a vehicle.
A rescue helicopter flew him to Liverpool Hospital in a serious condition, she said.
Investigations into the accident are ongoing.
